General Information Form

Natural Behavior Solutions will collect general information from you via email in order to provide you with insurance eligibility and programming needs.

Click "Intake Form" button above, download/complete and email back the form to mmartin@naturalbehaviorsolutions.com. A representative from Natural Behavior Solutions will be in touch within 2 business days. Preferred method of contact is email.

 

Methods of Payment

 

Insurance

Natural Behavior Solutions works with insurance companies. We are in-network and accept the following coverage: Tricare, Medicaid, CMS Title 21, Florida Blue, Blue Cross Blue Shield, CDC Waiver, Step Up for Students/EMA/Former Gardiner (PLSA), Aetna, Magellan, Staywell, and Sunshine Health. Upon completion of the General Information Form, we will need several pieces of information from you in order to receive an authorization for covered services:

• Diagnostic Report from psychologist

• A prescription for ABA Services from pediatrician (or primary physician)

• Copies front and back of Insurance card

 

Self Pay

We also take all forms of credit cards, personal checks and Venmo.
